Assistant Practitioner - Neighbourhood Team Community Nursing Our Community Nursing Teams are seeking an enthusiastic and dedicated Assistant Practitioner with a sound knowledge and experience of working with sometimes complex, frail and older people. CityCare's Community Nursing Service deals with a wide range of housebound cases, providing nursing care for people in their own homes. We are seeking an Assistant Practitioner with a wide range of holistic health and social care skills and experiences to complement our service, the teams are dedicated to preventing unnecessary hospital admissions and promoting independence and quality of life enhancement. You will be working alongside District Nurses, Community Matrons,Occupational therapists, Physiotherapist and a range of Health and Social Care Professionals. You will be required to undertake initial holistic assessments and follow up visits using a range of skills within the AP competency framework and under the guidance and supervision of the clinical staff. The hours of work will be 8.30-4.30 on a Rota basis.
