Band 6 Continuing Healthcare (CHC) Assessor (Permanent, Part-Time) Job Reference: 462-SOMCCG51-20 Employer: Department: Continuing Health Care Team Location: Yeovil, Somerset Salary: £30,401 to £37,267 per annum, pro rata NHS Somerset Clinical Commissioning Group (Somerset CCG) is looking to recruit a permanent, part-time Band 6 Continuing Healthcare (CHC) Assessor to join the wider Quality and Nursing directorate. This role will require you to undertake CHC and Funded Nursing Care (FNC) assessments and reviews on patients who may be eligible or who may already be in receipt of funding this will include those in receipt of fast track funding at end of life and those in receipt of a Personal Health Budget (PHB). The nature of the role will include providing and receiving highly complex, sensitive and contentious information, communicating unwelcome financial decisions to members of the public and using interpersonal skills to overcome barriers to acceptance where there may be hostility and antagonism. Liaison is an integral part of the role and will take the form of regular contact by telephone, face to face meetings, written and electronic communication. You will need to be a registered nurse, mental health nurse or an allied health professional with excellent assessment and interpersonal skills, experience in CHC is desirable but not essential. This post is based at NHS Somerset CCG HQ - Wynford House, Lufton Way, Yeovil, Somerset, BA22 8HR. However, staff are currently based from home due to the current Covid-19 pandemic.
