SECONDMENT OPPORTUNITIESAre you interested in an exciting opportunity with the Continuing Healthcare (CHC) Service in Berkshire West CCG?We are looking for three enthusiastic professionals with nursing, social work or allied health professional qualifications with drive and passion to join the Berkshire West CCG CHC Service and undertake quality continuing healthcare assessments for people with long term health conditions. The role is central to the delivery of Continuing Healthcare across Berkshire West through collaborative working across disciplines and professions with a primary objective of ensuring fair access to CHC with better outcomes and better experience for our patients.CHC Services assess and manage a diverse number of patients with long term conditions and provides excellent experience to professionals wishing to extend their knowledge of long term conditions, quality assessment and risk assessment processes as well as determining care packages and ongoing case management requirements.Working in a CHC Service equips professionals with knowledge and experience that will be invaluable to them in their future careers.THESE POSTS ARE FIXED TERM / SECONDMENT FOR A MINIMUM OF 12 MONTHS.
