Full Job Description

We have partnered with a leading not for profit Social Enterprise who provide innovative and patient focused primary care services to over 5 million patients, including GP led out-of-hours services, NHS 111, primary care, and secondary care support services.

They are seeking an Advanced Nurse Practitioner to join their team at the Primary Care Hub which has been running for over 10 years. You will be operating from a modern multi-purpose NHS site, and work closely with a multi-skilled and supportive team who continuously strive to ensure and maintain the highest standards of patient care and best practice.

Position Details:

• Job Title: Advanced Nurse Practitioner (ANP)
• Job Type: Full or Part-time; Shifts available Mon to Sun (operating hours - 8am to 6:30pm; Mon to Fri and 8am to 8pm; Sat/Sun)
• Location: Near Hastings, East Sussex

About You:

• Registered Nurse with NMC registration.
• Experience within a general practice or urgent care setting.
• Demonstration of innovation in practice/service development and understanding and experience of audit.
• Evidence of continuing professional development and education.

Key Duties

• Delivering primary care to non-registered patients by face-to-face consultation and/or telephone/video consultation.
• You will provide assessment, treatment, and diagnosis at first point of contact, and directly prescribe medication within your role as an independent prescriber.
• Coordinate care delivery, involving patients and their families, ensuring adherence to care plans and involvement in decision-making.
