ROLE: Clinical Pharmacist

LOCATION: Hailsham, East Sussex

HOURS: Permanent, Full or Part-Time Hours Available

Coben Healthcare is looking for a dedicated and compassionate Clinical Pharmacist seeking a fulfilling role within a forward-thinking and innovative Practice.
You will be supported by an experienced Multi-Clinical Team of GP Partners/Salaried GPs, ANPs, Nurses, MH Practitioners and more.

About You:

  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Experience as a pharmacist, demonstrated within a practice portfolio.
  • An awareness of common acute and long-term conditions.
  • May hold or be working towards an independent prescribing qualification.
  • Recognises priorities when problem-solving and identifies deviations from normal pattern and is able to refer to seniors or GPs when appropriate.
  • Excellent interpersonal and communication skills, and be organised, patient and empathetic.

Main Duties:

  • Undertake Long-term Condition Clinics (COPD/Asthma) and Medication Reviews
  • Conduct Patient Facing Medication Reviews including Care Home/Domiciliary and F2F
  • Manage a caseload of patients with common/minor/self-limiting ailments
  • Make recommendations for medicine optimisation and safety.
  • Support staff with prescription and medication queries as well as manage repeat prescriptions and monitoring tests.
  • Lead medicines optimisation and quality improvement.
  • Provide training on therapeutics and medicines optimisation and critically evaluate literature and ensure compliance with CQC standards
