Coordinator- Preparation for Adulthood
3-month interim contract to start immediately
London
£280 per day
Inside
IR35 (via umbrella)
Remote/ Hybrid working (2 days on site per week)- Remote working can
be discussed

Reporting into the SEND Team
Manager, the role of EHCP Coordinator is
to support the Local Authority and work with other services, such as
adult social care, to support the delivery of EHCPs using the
preparing for adulthood agenda.
Your role as EHCP Coordinator- Preparation for Adulthood:
- To carry out the casework associated with the statutory assessment process using the preparing for adulthood agenda.
- Attending Annual Reviews as appropriate.
- Supporting and coordinating Transfer reviews.
- Liaising with external stakeholders, such as, specialist schools as well as mainstream providers.
- To be an integral part of the SEND and inclusion team working collaboratively with colleagues across all departments and with partners.
Your experience as an EHCP Coordinator- Preparation for Adulthood:
- Excellent understanding of transitions into Adult Social Care
- Demonstrable experience with phase transfer in the post 16 sector, presenting to panels and have experience in writing EHC's using the preparing for adulthood agenda.
- Expert knowledge on SEND legislation.
- Must demonstrate a high level of operational experience in a Local Authority setting
