Intensive Dementia Outreach Worker

Full Job Description

My client is looking for an experienced Intensive Dementia Outreach Worker
to provide person-centred, high-quality community-based support to enable people with dementia and their carers to maintain their independence, choice, and control.

The worker will support group sessions at both the Dementia Hub in Leyton and Windmill Court in Chingford, including supporting with personal care, escort service users on minibus to sessions and undertake home visits to service users homes. They will be supporting service users with a range of activities to keep them stimulated and prove an opportunity for them to socialise.
An enhanced DBS is required for the role.
