Hours: 37 hrs

Meridian Business Support are working on behalf of a Healthcare group. They require A qualified band 6 Occupational Therapist. Experience is preferred in Intermediate care, rapid response and hospital discharge. You will be based in an Intermediate care service and working within patients own homes and in Intermediate care bed-based units.

Key Responsibilities:

• Be able to work both independently and as an effective team member alongside other professionals within health and social care services.
• Communicate regularly and effectively to maximise patient rehab potential and complete paperwork efficiently and comprehensively.
• Complete therapy assessments, carry out rehabilitation interventions, provide crisis interventions and attend multidisciplinary meetings to improve outcome for service users and reduce length of stay in this service area.
• To have a hands-on approach with rehabilitation goals and treatments and be able to complete and update care plans for rehabilitation care staff to follow.

Skills and Knowledge Requirements:

• Qualified Occupational Therapist
• 2-year post qualification experience in Occupational Therapy
• HCPC registered.
