Full Job Description

Phoenix PCN are recruiting a full time primary care network care home clinical pharmacist to work alongside a multidisciplinary, diverse team of health care professionals, providing support for prescription and medication queries.
 
Key responsibilities include:
 
  • Working alongside & fostering strong relationships with a multidisciplinary team across general practice to ensure the PCN is compliant with CQC standards.
  • Reviewing & making recommendations for the ongoing needs of patients facing long term conditions; ensuring they can get the best outcomes from their use of medicines.
  • Managing the use of medications with unplanned hospital admissions & high-risk patient groups.
  • Managing, reconciling & auditing medicines at discharge from hospital, working with patients, staff & relatives in our care homes alongside community pharmacists to ensure patients get the medicines they need and referred to the appropriate healthcare professional for their care.
  • Safely managing medicines for patients at high risk of harm & to managing product withdrawals for medicines identified in MHRA alerts.
  • Auditing compliance against NICE technology & assessment guidance and making recommendations to GPs for medicines that should be prescribed or require shared care.
About the Candidate
You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:
 
  • Completion of a degree in pharmacy and registration with the General Pharmaceutical Council.
  • Post-qualification experience in pharmacy (hospital, primary care or community), or working towards obtaining this.
  • Qualified independent prescriber, or working towards this.
  • Member of the Royal Pharmaceutical Society (RPS).
  • Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice.
  • Ability to demonstrate general pharmaceutical skills and knowledge in core areas.
  • Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions.
  • Ability  to demonstrate accountability for delivering professional expertise and direct service provision as an individuals.
  • Ability  to recognise priorities when problem-solving and identify deviations from normal patterns, referring to seniors or GPs with confidence, when appropriate.
  • Ability  to follow legal, ethical, professional and organisational policies, procedures and codes of conduct.
  • Ability to assist patients in decisions about prescribed medicines and supporting adherence as per NICE guidance.
  • Non-judgemental attitude and ability to be flexible and adaptable.
  • Self-motivated with excellent communication skills.
  • Understand the need for patient confidentiality.
About Us
The Phoenix PCN was formed in 2018, and both practices have been working effectively together ever since. The PCN staffing is expanding, and currently employs clinical pharmacists, paramedics, and a social prescriber. 
 
Longfield Medical Centre which is a seven partner dispensing and training practice located within the town of Maldon, with a list size of approximately 17,000 patients. Tollesbury Surgery is a two partner practice located in the rural village of Tollesbury, and have approximately 5000 patients.
