Are you passionate about public health and creating healthier communities? The Suffolk Health Department is seeking a flexible and motivated Public Health Nurse ( PHN ) to lead the Medicaid Long-Term Services and Supports ( LTSS ) program in Suffolk. The PHN will work in our City of Suffolk office and will be conducting home screening visits in partnership with Suffolk’s Department of Social Services. This position promotes public health and well-being for Suffolk residents, particularly the older adult population. The PHN may assist in facilitating relationships among the Western Tidewater Health District, community based health care providers and community members and is knowledgeable of social and economic conditions related to public health and public health nursing. The PHN will be able to display sound, independent nursing judgments; analyze and problem solve and establish priorities, as well as establish and maintain effective working relationships with the community.

Minimum Qualifications
Current RN License in the State of Virginia. Must have CPR certification or ability to obtain certification. Knowledge of the principles and practices of nursing, community resources and organizations, social and economic conditions, program administration and complex medical treatments. Considerable skill in monitoring the side effects of complex treatments. Must have previous experience with home-visiting, community based nursing, public health or compatible experience or training. Ability to communicate with staff, patients, families and public health nursing management. Ability to work independently, problem solve and work collaboratively with other agency staff as well as the ability to set priorities and work with minimal supervision. Must possess critical thinking skills. Proficiency in computer functions including keyboarding, email, Microsoft Office and experience with data entry. Required to maintain a valid driver’s license and adhere to VDH’s driver safety policy.

Preferred Qualifications
BSN , degree in health and human services or MPH . Case management experience; home visitations, knowledge of public housing communities, experience working with the older adult population, public health experience and considerable nursing experience preferred. At least one year of med-surg or inpatient hospital experience preferred. Knowledge of LTSS policies and procedures. Experience with long-term care screenings, DMAS or Virginia Medicaid processes preferred.
