Full Job Description

As a Gateway Coordinator you’ll engage with customers as they access the Gateway Service for homelessness support. You’ll make sure that they receive support and advice that is reactive and tailored to them, to prevent and reduce homelessness. You’ll also be the first point of contact for the service for referrals and queries from other organisations.

Job requirements

  • Work as part of the Destination:Home Gateway Team to promote Concrete’s services to potential customers, stakeholders and landlords.
  • Work in partnership with Destination:Home partners Adullam, Mind, Changes and WALK to help deliver a 24/7 service that helps customers thrive in their homes.
  • Be the first point of contact within the partnership for customers, other professionals and organisations.
  • Work with referrers to make sure all information, including about risk, can be captured within the referral and you fully understand the customer’s needs.
  • Provide support to adults with complex needs such as substance misuse, homelessness and mental health. You’ll provide support around their referral and housing options.
  • Be an ambassador for Concrete and communicate our mission to customers and stakeholders.
  • Attend regular internal and external multiagency meetings.
  • Work with the local community and other agencies to build positive relationships and promote the service.
     

What we’re looking for

  • Someone educated to GCSE Level or with a good level of maths and English.
  • Someone with experience of supporting adults with complex needs such as substance misuse, homelessness and mental health, or transferable skills you can bring to the role.
  • Great communication, problem-solving, organisational and time-management skills.
  • Someone with genuine compassion for people affected by homelessness, with an understanding of the barriers they face.
  • DBS check.
