Patient Flow Clerk

An exciting opportunity has arisen to be a part of our pivotal Patient Flow Team.

We are seeking highly motivated individuals who like to work in a fast paced environment to join our busy 24/7 Patient Flow Team, to assist in the day-to-day operational management of patient flow across the Trust, which covers two sites.

The successful candidate will be required to work closely with clinical and operational teams to facilitate the optimal use of beds across the organisation.

The post demands excellent communication skills, as it involves internal and external reporting to be completed pre and post operational capacity meetings, and excellent customer care service skills.

Flexibility and an ability to adapt to change quickly is essential to the role, as is the ability to perform in a high pressure environment.

The post involves a high level of calls, administration duties, internal and external reporting, booking patient transport and use of hospital IT systems, therefore ability to work well under pressure and meet deadlines is essential.

The post holder will be required to work on a rotational basis including nights, weekends and holidays.

Hours cover early, late and night shifts.

• Comply with Bed Management protocols.
• Converse directly with General Practitioners regarding the admission of patients and organise emergency transport via ambulance control when required.
• Provide accurate and concise information for bed meetings (5 scheduled throughout the day).
• Provide daily sitrep to information team.
• Collate data from information team and WIC and submit sitrep to NHS England weekends/Bank Holidays and OOH.
• Independently work within the admission areas, monitoring patient flow.
• Liaise with ward staff and gather concise information regarding actual or potential discharges, next day discharges, internal hospital delays and potential outliers.
• Liaise closely with the Infection Control Team regarding the infection status of the hospital and any outbreaks in the community.
• Update the Capacity Management System throughout the day and night.
• Circulate bed escalation via email to hospital managers.
• Assist in achieving the 4-hour target in A&E.
• Effectively manage all telephone enquiries.
• Communicate with Consultants and Bed Managements at other trusts regarding safe and timely transfers.
• Responsible for the collation and continual update of Bed Managements statistics.
• Photocopy forms and documentation for department.
• Demonstrate own duties and provide On-The-Job Training to new members of staff.

• 5 GCSEs or equivalent
• NVQ LEVEL 3 customer care or equivalent. ECDL or equivalent
• Clerical experience or experience of dealing with the general public
• Experience of working as part of a team

Discharge Coordinator

The Discharge Coordinator team are responsible for the management of complex discharges of patients referred to the Transfer of Care Hub. Discharge Coordinators work with patients, families and members of the Multi-Disciplinary Team (MDT) to facilitate safe, appropriate and timely discharge of this patient group through assessment of care needs to enable identification of a facility that can meet these needs.

This is a rare and exciting opportunity for an experienced nurse to join our Discharge Coordinator team which is part of the Transfer of Care Hub. As a Discharge Coordinator you will be responsible for the assessment and discharge planning of patients with complex discharge needs including those that are nearing the end of their life and are in the terminal phase of their illness.

• Registered general nurse (RGN) educated to degree level (or equivalent experience)

Senior Therapy Assistant

We are looking for an enthusiastic, friendly and compassionate therapy assistant with a passion for helping adults to live better with the management of complex and chronic pain conditions.

The Wirral Chronic Pain Service is a multidisciplinary team including doctors, physiotherapists, psychologists, an occupational therapist and a pharmacist. You will work within the pain service to support the smooth running of the team by performing a range of clinical, clerical, administrative, housekeeping and departmental tasks.

The role will involve working directly with the pain service team members and require confidence in delivering delegated management plans and activities with an individual caseload and also as part of our group sessions.

You will also be involved in an exciting development working with third sector and volunteer groups. We welcome applications from effective communicators as this post will involve working closely with multidisciplinary team members, service users and community organisations.

• Communicate effectively within the Therapy Team and other members of the multidisciplinary Team.
• Establish a rapport with patients to help gain their compliance with the exercise programmes.

• Prepare the environment for the therapy session – including clearing the area of obstructions, preparing equipment (e.g., treadmill, static bicycles), manoeuvring hoists and other therapy equipment.
• Assist the Therapist during a treatment session/ intervention.
• Work with the Therapist to assist in treatment sessions, when required, in a wide range of activities including exercises and practising activities of daily living, and accompany the Therapist on home visits.

• Must have a relevant Level 3 NVQ or an equivalent level of experiential knowledge and experience in an appropriate area.
• A minimum of 2 years recent experience in a related health care setting.

• Be computer literate, able to use hospital computer systems and databases for audit following appropriate training.
• Show initiative and manage own workload, but seek advice when appropriate.

The post holder will act as a professional and clinical leader in the area of AMS across the Wirral health economy to deliver improvements in the utilisation of antimicrobials. They will work in partnership with Clinical Leads, Microbiologists, Public Health Experts, Managers and Pharmacy Leads to continually improve the medicines optimisation for patients requiring antimicrobial therapy.

The post holder will need to work with others to develop a strategy to support the AMS agenda and reduction in AMR across Wirral in collaboration with key internal and external stakeholders that fosters a culture for quality improvement and continuous learning.

We are a large department, employing 95 pharmacists and 47 MMTs delivering services to the acute hospital and primary care network (PCN). High quality, proactive input is expected by our clinicians, nursing staff and patients; as such over half of our pharmacists are independent prescribers, actively using this qualification.

• Vocational Masters Degree in Pharmacy (or equivalent)
• Registered with the General Pharmaceutical Council
• Extensive post registration experience within the NHS including recent senior service leadership role within NHS pharmacy practice
• Management and/or leadership qualification or relevant experiential learning within the NHS in a senior position within pharmacy.
• Significant experience in project management including leading on major change projects
• Evidence of post‑qualification continuing professional development
