Overview

The post holder will be a key member of the Sexual Health, HIV and Contraception service and will have an important role in ensuring delivery of the services within Sexual Health Hounslow. Operating within a fast-paced environment, the role involves providing comprehensive operational and administrative support to the Sexual Health Hounslow team. This role requires a professional operational lead with a proactive and composed approach, capable of multitasking effectively and can contribute strategically to utilisation and development of the service.

Responsibilities
  • Managing the Deputy Administrative Lead and Patient Care Coordinators across three sites. The post holder oversees the running of the three reception areas to maintain continuity of cover at all times. The post holder will be responsible for recruitment and training of new administrative staff, coordinating the day-to-day management of the administrative and reception team and creating rotas according to leave.
  • Overseeing the implementation of clinic design and changes to maximise slot efficiency, reducing DNAs and promoting attendances. Assess priorities around workload in conjunction with the Clinic Manager and wider strategic priorities. Attend virtual and in-person meetings within Sexual Health Hounslow and across the HIV, Sexual Health and Dermatology Directorate when required.
  • Operational:
    • To ensure collaborative working and clear communication pathways between clinical and administrative staff.
    • To work with the Clinic Manager to ensure quality standards are being met and improved upon including service user satisfaction (Friends and Family test).
    • Manage administrative resources in conjunction with the Clinic Manager.
    • Place requisitions for orders using the Trusts system (IT, Procurement, Estates).
    • Attend and participate in meetings within Sexual Health Hounslow and across the Directorate.
    • To be the first point of contact for all environmental issues including IT.
    • Ensure the clinic is clean and the clinical and administrative staff are in a fully functional working environment.
    • To ensure that the Clinic Manager and Clinic Lead are kept informed of any potential or emerging issues and to participate in monthly one-to-one meetings.
    • To maintain and update room usage including the booking of the Seminar Room.

Job Overview

A detailed work programme and a job plan will be arranged and agreed by the appointee with the Service Director in Medicine and the Chief Executive of the Trust.

The post holders will undertake fixed clinical and non-clinical sessions according to an agreed timetable with other members of the Consultant Team.

The post will be divided into a ratio 7.5:2.5 direct clinical care (DCC) / supporting programmed activities (SPA). Specific SPA objectives and outcomes will be agreed with the Service Lead within 3 months of appointment.

The current consultant work pattern provides hands on Consultant ‘shop floor’ presence from 09:00 to 21:00 Monday to Friday, 10:00 to 19:00 Saturday and Sunday to directly lead and guide the team to provide a high standard of clinical care for patients attending the Paediatric Emergency Department. There are no on call commitments as part of this post. This will change as the PEM consultant group continues to grow.

The Department is busy, seeing over 169,000 patients per year (20% paediatric) in conjunction with the Urgent Care Centre, achieving a high clinical standard of care and delivering the 95% 4 hour target. This is achieved through working as a close, co-operative team.

Responsibilities
  • To be responsible for ensuring patients experience an efficient patient pathway in keeping with national Key Performance Indicators except where the patient’s condition prohibits this.
  • Supporting the governance programme in the Paediatric Emergency Department
  • Trainee supervision, including WBPAs
  • The organisation and supervision of minor procedures in the Emergency Department
  • To effectively communicate decisions on continuing care to the Patient’s GP

Departmental Information

The Emergency Department (ED) at West Middlesex Hospital sees 169,000 patients a year of which approximately 20% of these are children, providing the service to a large multi‑ethnic urban population. We are currently in the top ten preforming emergency departments in the country.

Paediatric Emergency Department (PED) falls within the Emergency Medicine Directorate and the paediatric services falls within the Children, Young People’s and Neonatal Directorate. The overall approach of the paediatric service is family and child centred with multi‑professional teams including paediatric nurses, psychologists, dieticians, physiotherapists, social workers and the Hospital School working together.

The dedicated Paediatric Emergency Department and the adjoining UCC, is open 24/7, which treats approximately 42,000 children per year with 24,000 are seen and managed in PED. It is co‑located with the main Emergency Department with a separate audio‑visual waiting room.

The PEM team is comprised of 4 PEM consultants, PEM registrars, Advanced Care Practitioner, CT3/JCF/FT2 doctors who have designated shifts in the PED, CESR doctors competing their secondments in PEM, as well as being entirely staffed by paediatric nurses.

Our department is a 9 bedded space, including an HDU space, a mental health room, a separate isolation room and a paediatric dedicated bay in the 4 bedded resuscitation area.

The Paediatric Emergency Departments is undergoing major service and workforce review and developments. The department is establishing a dedicated junior paediatric emergency team including advanced nurse practitioners.

PACS is installed and the department is very well supported by the Radiology Department with ready access to all investigations and prompt reporting. The department is fully computerised for patient attendances, investigations (radiology & pathology) and results and discharge summaries. Within the department we have Point of Care Testing for FBC, U&E and ABG and several ultrasound machines.

The department is a member of PERUKI (Paediatric Emergency Research in the UK & Ireland) and is commencing it research portfolio, alongside Chelsea and Westminster Emergency Department support with its strong track record. There is a strong track record of supporting trainees to develop projects into presentations at national and international conferences.

There is an active educational program cross site, both undergraduate and postgraduate, is driven by the emergency and paediatric departments creating a wide variety of learning opportunities including weekly simulation and monthly emergency training days.

Teaching duties will include medical students, sessions for the FY2s and middle grade teaching including preparation for RCPCH/FRCEM.

The post holder will be required to participate in the Trust and Directorate’s Clinical Governance activities, with a commitment to audit, annual appraisal for themselves, revalidation and CPD, and regular appraisal and mentoring of other team members.

It is expected that the post holder will engage in specific management tasks and contribute to monthly departmental meetings (sub‑directorate and Clinical Effectiveness) and hospital committees in agreement with the Service Lead and the Directorate Policy Board.

Overview

Our Coronary Care Unit (CCU) and Cardiology ward (Syon 1) is a 30 bedded ward, which comprises 6 CCU beds and 22 cardiology and heart failure step-down beds. An exciting opportunity has become available for a Sister or Charge nurse to support the Ward Manager in leading the nursing team. The post holder will be expected to provide a high standard of care including assessment, planning, implementation and evaluation of individualized, holistic patient care in conjunction with the multi-disciplinary team. As a proficient practitioner in cardiac care, the post holder will be expected to act as a role model to members of the nursing team. He / she will provide effective mentorship and preceptorship, participating in the education and training of nursing and non-nursing staff. The post holder will also take charge of the Coronary Care Unit as required, liaising closely with the Ward Manager and Modern Matron and will assume responsibility of providing safe and effective nursing care.

To provide exceptional patient care which is underpinned by evidence based practice and Trust values. To work in partnership with the patients’ family and friends and actively involve them in the caring process on behalf of the patient where agreed. To ensure safe allocation of nursing staff caseload depending on nursing staff skill mix. To communicate effectively with patients/relatives/carers/ and all members of the multidisciplinary team. Demonstrate high standards of nursing care, reflecting the ethos of NMC Code of Conduct. To develop and maintain specialist knowledge of nursing practice relevant to the clinical area. To undertake the responsibilities associated with being a mentor and preceptor reflecting the beliefs about professional nursing practice and patient care. Demonstrably contribute towards the continuing development of professional nursing practice in the department. To conduct oneself in a professional manner and be a role model to others at all times.

Roles and responsibilities

Clinical:

  • To develop further in the area of clinical expertise.
  • To be up to date on clinical practice and implement and change practice of self and others as appropriate.
  • To be an expert in the designated basic clinical skill set and to teach, inform and coach others as appropriate.
  • To positively contribute to research and audit initiatives.
  • To ensure patients undergoing procedure for the cardiac catheter lab are prepared as per protocol in a timely manner.
  • To facilitate and organize occasional transfer to other tertiary hospital, on the North West London Area, for patients needing complex invasive cardiology interventions and procedures.
  • To manage the flow of patients within the clinical area taking into consideration levels of care and patient acuity.
  • Always adhere to infection control policies and procedure and be able to challenge staff not following IPC guidelines.

Leadership:

  • To co-ordinate the Cardiology unit on a regular and on-going basis.
  • To support less experienced staff to set priorities and support them to flourish their clinical skills and experience.
  • To take a lead in specific areas of delegated responsibility within the ward team (clinical & non-clinical).
  • To assist Team Leaders in the development of others and deputises appropriate.
  • To supervise temporary / bank staff.
  • To act as a mentor for Band 5 nurses and nursing students (as sign-off mentor).
  • To empower others to make decisions.
  • To be able to challenge poor practice and support staff to follow protocols and procedures.

Educational:

  • To contribute towards staff development within cardiology by means of teaching in the classroom and in the clinical setting.
  • To contribute towards the knowledge of other healthcare professionals within primary and secondary care through teaching and presentations.
  • To develop and maintain own clinical skills and expertise by attending relevant study days/conferences where appropriate.
  • To participate in the orientation of new staff and students.

Professional:

  • To act in accordance with the NMC Code of Conduct.
  • To exercise sensitivity and diplomacy regarding role boundaries with other healthcare professionals across the Acute Trust and local Primary Care Trust.
  • To engage in personal clinical supervision.
  • To participate in own annual appraisal with regular review with line manager, identifying own development needs and appropriate learning that relate the role.

Audit and Research:

  • To facilitate and participate in the collection of audit data and outcome measures to demonstrate effectiveness of the service provided, and to highlight areas for further development.
  • To participate in local and Trust audits such as Infection Control Audit and other Quality Improvement Audits.
  • To present results of audit to the Trust and cardiac directorate.
  • To obtain, evaluate and implementation findings from research and other information to facilitate best practice.

Communication:

  • Ensure all communication, which may be complex, contentious or sensitive, is undertaken in a responsive and inclusive manner, focusing on improvement and ways to move forward.
  • Ensure all communication is presented appropriately to the different recipients, according to levels of understanding, type of communication being imparted and possible barriers such as language, culture, understanding or physical or mental health conditions.

Responsibilities
  • Provide a customer focused service dealing with all patients or visitors in an efficient, polite and courteous manner.
  • Support the delivery of a high quality healthcare service ensuring that all patients receive a high standard of service delivery.
  • Handle queries from patients/relatives confidentially and sensitively; refer clinical related queries to appropriate staff.
  • Interact with patients and relatives in distress in a sensitive and empathetic manner.
  • Take and relay, as appropriate, telephone enquiries and messages, using initiative and prioritising skills when disseminating information.
  • Update patient details on Soliton/Cerner as appropriate, checking all demographic details on arrival or departure.

The post holder will provide a comprehensive patient focused administrative service, on a day-to-day basis to a multi-disciplinary team based in our AEC Department . Roles and responsibilities

Clinical Office:
• Liaise with consultants and other admin staff regarding patient appointments, referral letters, discharge summaries and health records
• Contact other departments and teams as necessary relating to the patient care pathway and service delivery
• Support the Team Leader and practitioners with the smooth running of clinics and procedure lists.
• Liaise with other hospitals and GPs regarding patient care pathways
• Prepare for clinics ensuring all relevant documentation is available and notes are prepared
• Use Trust’s computer system, to generate clinic lists and help locate missing notes
• Use of the case note record tracking(CRT) system to electronically
register/move/transfer/locate medical case notes
• Prepared all externally transcribed correspondence for signature and despatch within the agreed Trust timeframe, escalating where necessary.
• Match all correspondence typed by the managed transcription service with correct patient and clinician, any amendments are made, and letters dispatched and filed in medical record.
• Correct any mismatches by reference to medical record, original dictation, communication with the transcription service and if required clinical staff.
• Provide any specific advice to patients upon booking-in, in preparation for their appointment
• Ensure all patients are added to the Trusts PAS system before their appointment
• Type urgent letters and reports as required
• Register new referrals, and ensure all patient information is up to date and complete
• Book outpatient appointments
