Main ResponsibilityCommunity based role:
- Participate in assessment and care planning of patient care in conjunction with the Team Manger and support of specialist team members.
- Implements and evaluates care and treatment in agreement with the patient and as defined in the care plan – escalates any deviation from the care plan or change in condition to the Team Manager /GP accordingly
- Communicates effectively with patients, carers and all members of the multidisciplinary team
- Prioritises daily work in line with clinical / patient need
- Maintains accurate and legible patient records in line with professional and organisational requirements – this includes electronic patient record system and any paper records
- Implements and works to ensure evidence-based care / best practice
- Actively participates and contributes to team meetings and daily handover
- Initiates health promotion activities with individuals, patients and carers
Ward based role:
- Maintain a high standard of patient focused care
- Maintain a safe and clean working environment
- Provide assessment, planning, implementation and evaluation of nursing care for patients, ensuring accurate written records are kept
- Communicate with patients and carers to ensure they have access to appropriate
- Promote and maintain positive relationships between all staff involved in the care of the patient

Join our dynamic and friendly community nursing administration team, working closely with the wider Integrated Care Team. In this varied role, you’ll be responsible for answering telephone calls from patients and healthcare professionals, booking new and follow-up appointments, and uploading referrals, clinical documents, and photographs to the Clinical System (EMIS).
You’ll need to be confident, flexible due to working shifts and able to use your initiative when responding to patient enquiries, with guidance available from the Senior Community Nurse. Strong organisational skills are key, as you'll need to prioritise tasks effectively while helping to maintain a well-run, welcoming office environment. Above all, a polite and respectful manner when dealing with patients and their representatives is essential.
This is a part-time role of 22.5 hours per week, working three days 7.5 hours a day between Monday to Friday. Shifts cover the service hours between 8:00 am and 8:00 pm, with early shifts from 8:00 am to 4:00 pm and late shifts from 12:00 pm to 8:00 pm. Generally, working to a 5 week rolling rota which helps plan your diary, although this can change due to holidays.
Main ResponsibilityAs an administrator you will:
- To provide comprehensive, confidential and effective administration to the community nurses working alongside the Integrated Care Team.
- First point of contact for dealing with telephone calls from patients and health professionals in a courteous and timely manner. Relaying all messages in detail, both written and verbal messages then escalating to nursing staff as required.
- Monitoring the SPA email inbox, manage the community nursing referrals, categorising emails, uploading on to the IT System (EMIS), this can include graphic wound photos. Booking appointments for patients.
- Telephone calls with patients, carers, and health professionals maintaining positive working relationship, often dealing with complex and difficult phone calls. Displaying a courteous, empathetic and effective communication skill, aware that information may be sensitive. Monitor telephone reporting tool to ensure telephone lines are active and closed at the end of the day.
- Administrative duties include, booking meeting rooms, tracking unplanned visits, updating daily sheet and completing friends and family.
- Continence assessment – Discuss with patient or carer, if meets criteria completes an order form and email to Tena.
- Disseminates service information to staff across the relevant team / designated area as requested using various means of communication: telephone or e-mail.

The Ideal CandidateWe are looking for someone with;
Qualifications and Training
- Good standard of general education with ‘O’ level, GSCE (or equivalent) in English
- IT / computers / typing
Experience
- Previous experience using computer packages, databases, word, excel, microsoft access, power point
- Experience of working in an office environment and diary management, filing, emails etc.
- Relevant experience in working with people
- Able to manage occasional exposure to distressed members of the general public / customers
- Experience of setting up and operating administrative and organisational procedures required for efficient office operating processes

As a Nursing Associate within the prison service, you will work as an effective member of the Pharmacy team in providing healthcare services, including medication administration to the patients. Main Responsibility- Administer Medication in accordance with the local SOPs.
- Assist with the implementation and development of pharmacy support to all the residential wings in the prison which includes, where appropriate, advising patients on the correct use of medication.
- Maintain clear and comprehensive, signed and contemporaneous records both written and electronic.
- Regularly monitor and update medication charts, removing out of date. prescriptions and ensure prescription renewals from prescribers are ordered timely.
- Reconcile medicines on a daily basis for all prisoners who are admitted to the prison directly from the Courts or via internal transfer from another prison.
- Undertake risk assessments of prisoners relating to ‘In-Possession’ medication.
- You will collect orders from suppliers from the Main Gate and arrange collection of return. You will participate in dispensing of prescriptions.

Experience in helping residents with different health needs is important too, as is a successful track record of managing safeguarding investigations and clinical risk assessments. Background –
Menlo Park are working with a very well respected private healthcare provider looking for a Specialist Nurse Prescriber to work on one of their prestigious client sites in Central London. They are looking for an experienced Nurse Prescriber, able to see a mixture of acute and basic routine presentations as well as treatment room duties such as travel vaccinations, flu jabs, ear irrigations, smears, etc. It is a very varied role and they need someone who is already an independent prescriber. This is a full-time role (37.5 hours), working to 15 minute appointments and working alongside other experienced clinicians such as GPs, Occupational Health Practitioners, Advisors, etc. You will be the sole nurse on site so need to be comfortable working autonomously.
Salary – £49,000 - £8,000 per annum DOE + Private Pension + 5 weeks annual leave + Bank Holidays + Indemnity + Private Healthcare + Critical Illness Cover + Enhanced Maternity/Paternity Pay + Other great benefits
