This is a fully remote position, offering the flexibility to work from anywhere within the UK while managing a portfolio of clients and care teams. You will play a pivotal role in ensuring the highest standards of care are met, coordinating complex care packages, supporting care staff, and liaising with healthcare professionals and families. Your leadership will be instrumental in driving excellence and positive outcomes for vulnerable individuals.

Responsibilities:
  • Oversee and coordinate the provision of high-quality care services to a caseload of clients, ensuring individual needs are met comprehensively.
  • Develop, implement, and regularly review personalized care plans in collaboration with clients, families, and multidisciplinary teams.
  • Provide clinical guidance, support, and supervision to care staff, fostering a culture of continuous learning and development.
  • Conduct comprehensive assessments of client needs, including risk assessments, to inform care planning.
