Key Responsibilities:
- Support the administration of QOF and Quality Contract submissions, ensuring accurate and timely data entry.
- Work closely with clinical teams to support chronic disease management programmes (e.g., diabetes, hypertension, COPD).
- Monitor recalls, reviews, and patient registers to ensure contract requirements are met.
- Use EMIS Web to extract reports, update patient records, and manage appointment and recall systems.
- Communicate effectively with patients, clinicians, and external stakeholders.
- Assist with the organisation of clinics, sending invitations, and tracking patient compliance with annual reviews.
- Maintain accurate records in line with data protection and confidentiality policies.
- Provide general administrative support, including managing correspondence, scanning documents, and filing.
