• Assess, plan, implement, and evaluate comprehensive nursing care for patients in their own homes and community settings.
• Manage a complex caseload, prioritizing patient needs and ensuring timely interventions.
• Provide specialist nursing care, including wound management, palliative care, chronic disease management, and medication administration.
• Work collaboratively with multidisciplinary teams, including GPs, social workers, and allied health professionals, to ensure seamless patient care.
• Educate patients, families, and caregivers on health conditions, treatment plans, and self-management strategies.
• Supervise and mentor junior community nurses and healthcare support workers, fostering their professional development.
• Contribute to the development and implementation of policies, procedures, and quality improvement initiatives.
• Maintain accurate and up-to-date patient records using electronic health record systems.
• Act as a patient advocate, ensuring their needs and wishes are respected and upheld.
• Participate in on-call rotas as required.

• Registered Nurse (RN) with current NMC registration.
• Significant post-registration experience, with substantial experience in community nursing or a related field.
• Evidence of further professional development, such as a degree or relevant post-graduate qualifications (e.g., in District Nursing, Palliative Care, or Chronic Disease Management).
• Proven leadership and mentorship skills.
• Excellent clinical assessment, decision-making, and problem-solving abilities.
• Strong understanding of current nursing practice, healthcare policies, and relevant legislation.
• Proficiency in using electronic patient record systems.
• Excellent communication, interpersonal, and teamwork skills.
• Full UK driving licence and access to a vehicle for community visits.
