  • Maintain accurate and confidential client records, documenting interventions and progress.
  • Collaborate effectively with other professionals, including social workers, healthcare providers, and local authorities, to ensure holistic client care.
  • Advocate for clients' needs and rights within the community and with external agencies.
  • Respond to crisis situations with appropriate interventions and support.
  • Contribute to the development and improvement of community support services.
Qualifications:
  • A genuine passion for helping others and a strong understanding of social issues.
  • Previous experience in a care, support, or community-focused role is highly desirable.
  • Excellent communication, listening, and interpersonal skills.
  • Ability to empathize with individuals from diverse backgrounds and experiences.
  • Strong organizational and time-management skills, with the ability to manage a caseload effectively.
  • Proficiency in basic IT skills for record-keeping and communication.
  • Ability to work independently and as part of a dedicated team.
  • A willingness to undertake relevant training and professional development.
  • A relevant qualification such as a NVQ/QCF in Health and Social Care or equivalent is advantageous.
  • Must be able to work within the designated geographical areas of London .
  • Flexibility to work varying hours, including some evenings and weekends as required by the needs of the community.
