We are seeking an experienced Medical Director to guide the clinical team and shape the future of care. • Lead and manage a team of community nurses and healthcare assistants providing palliative care.
• Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
• Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and the multidisciplinary team.
• Administer complex medications and treatments, including pain management and symptom control.
• Provide expert advice and support to patients, families, and carers on all aspects of palliative care.
• Liaise effectively with GPs, hospitals, hospices, and other healthcare professionals to ensure seamless care coordination.
• Educate patients and families on disease progression, treatment options, and self-care strategies.
• Maintain accurate and up-to-date patient records in compliance with NMC and organizational standards.
• Participate in case conferences, team meetings, and professional development activities.
• Ensure adherence to infection control policies and procedures.
• Mentor and support junior nursing staff, fostering a positive and learning environment.
• Champion best practices in palliative care and contribute to service development and improvement initiatives.
• Manage caseload effectively, prioritizing urgent needs and ensuring timely interventions.
• Provide emotional support and bereavement counseling to patients and their families.

• Registered Nurse (RN) with current NMC registration.
• Significant post-registration experience in palliative care, with demonstrable leadership responsibilities.
• Advanced clinical skills in symptom management and end-of-life care.
• Evidence of continuing professional development, including relevant postgraduate qualifications (e.g., Specialist Practice, Palliative Care).
• Excellent communication, interpersonal, and listening skills.
• Ability to work autonomously and as part of a dynamic team.
• Strong assessment, planning, and evaluation skills.
• Proficiency in IT skills, including electronic patient record systems.
• Valid UK driving license and access to a vehicle for community visits.
• A compassionate and empathetic approach to care.
• Commitment to continuous learning and professional growth.
• Experience in mentoring or preceptorship of junior staff.

• Provide expert specialist nursing care to patients requiring palliative and end-of-life care, focusing on pain and symptom management, and psychosocial support.
• Conduct comprehensive holistic assessments of patients and their families, identifying needs and developing individualized care plans.
• Act as a primary point of contact and advocate for patients and families, ensuring their wishes and preferences are respected.
• Collaborate effectively with physicians, social workers, therapists, and other healthcare professionals to ensure coordinated and integrated care.
• Offer expert advice, education, and support to healthcare professionals, patients, and families regarding palliative care principles and practices.
• Lead and participate in case conferences and multidisciplinary team meetings to discuss complex patient cases.
• Develop and deliver educational programs and training sessions for nursing staff and other healthcare professionals on palliative care topics.
• Contribute to the development, implementation, and evaluation of palliative care services and policies.
• Engage in research activities and stay current with the latest evidence-based practices in palliative care.
• Maintain accurate and timely patient records in accordance with professional standards and organizational policies.

• Supervise and mentor junior nursing staff involved in palliative care.

• Registered Nurse (RN) with significant post-registration experience.
• Master's degree in Nursing, Palliative Care, or a related field.
• Advanced specialist qualification or extensive experience in Palliative Care Nursing.
• Proven experience in conducting holistic patient assessments and developing care plans.
• In-depth knowledge of pain and symptom management techniques relevant to palliative care.
• Excellent communication, interpersonal, and counseling skills.
• Demonstrated ability to work effectively within a multidisciplinary team.
• Experience in education, mentorship, and service development.
• Strong understanding of ethical issues and psychosocial aspects of end-of-life care.
• Current NMC registration and relevant professional certifications.
• Willingness to travel between clinical sites as needed within the London area.
