We are seeking a committed and experienced Hospital Discharge Social Worker to join our team on a short-term contract. The post holder will play a vital role in supporting patient flow from psychiatric units , helping to relieve increasing hospital discharge pressures and ensuring timely and safe transitions from hospital to the community.
This role also includes supporting step-down arrangements and assisting localities with complex care discharges for patients aged 65 and over , ensuring that individuals receive the right care, in the right place, at the right time.
