Requirements

ROLE PURPOSE

We run a range of services and our ANPs work across 3 key services:

-    Additional Access service in the afternoons, Mon-Fri.

-    Enhanced Access service in the evenings, Mon-Fri and Saturday day time.

-    eHub is a digital same-day service for our 32 practices. ANPs speak to and possibly see patients assigned to them by our senior clinical decision maker who triages all the e-consultations we receive

-    We run research clinics, where we see and screen patients who are eligible for our studies.

-    Furthermore, we employ Nurse Associates and ANPs may be involved in the training and supervision of these staff.

  • The post holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice and other clinical settings.
  • The post holder will commit to demonstrating critical thinking in the safe clinical decision-making process. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

KEY TASKS AND RESPONSIBILITIES

Clinical

  • Working across the following specialities; Additional Access, Enhanced Access, eHub, and research 
  • Triage urgent care requests if required, consulting with patients or directing them appropriately.
  • Provide advanced nursing care to patients, including assessment, diagnosis, treatment, and follow-up.
  • Conduct holistic assessments on patients and ensure appropriate care and action plans based on their needs are clearly recorded and documented.
  • Undertake regular patient reviews.
  • Plan patient discharges safely and effectively, including use of independent prescribing in respect of providing medications on discharge.
  • Following assessment where appropriate/necessary and with the patients’ consent refer appropriately to other services and agencies.
  • Perform and interpret diagnostic tests, such as laboratory and radiological tests.
  • Prescribe and administer medications in accordance with the law and established protocols.
  • Collaborate with the healthcare team, including physicians, nurses, and allied health professionals, to ensure comprehensive patient care.
  • Maintain accurate patient records, including medical histories, treatment plans, and progress notes.
  • Educate patients and their families about their health conditions and treatment options.
  • Stay up-to-date on the latest medical research and guidelines relevant to the clinical specialty.
  • Maintain compliance with all relevant clinical standards and regulations.

Two year’s post qualification experience – consideration given to ANPs with primary healthcare experience (E)

▪ Advanced Clinical including Physical Examination Skills (E)

▪ Advanced Communication Skills (E)

▪ Demonstrate previous successful ability of working at Advanced Practice Level (E)

▪ Leading and Managing change in collaboration with others (E)

▪ Partnership working across sectors (E)

▪ Clinical Leadership in a Multi-Professional Service (D)

▪ Ability to plan and prioritise care with advanced decision-making skills (E)

▪ Demonstrate evidence of delivering formal and informal teaching (D)

▪ Previously worked within Primary care setting (D)

▪ Research - history of previously undertaking research or can show how it is used appropriately to guide your practice. (E)

▪ Excellent communication skills, multi-agency and multi-disciplinary (E)

▪ Applied pharmacology and evidence-based prescribing. (E)

▪ Leadership and team working abilities. (E)

▪ Effective IT skills, competent in Microsoft office, Teams, Sharepoint and SystmOne (D)

▪ Critical evaluation skills (E)

▪ Clear understanding of the systems of clinical governance and quality assurance, specifically relating to advanced practice (E)

▪ Ability to direct and co-ordinate programs of care, able to work both collaboratively and autonomously (E)
