Requirements
Outline of key responsibilities for the Support Officer…
- Provide housing-based support to older, vulnerable and disadvantaged people, ensuring support is person-centred, flexible, and outcome-focused, tailored and adapted to each to meet the needs of the individual and children. This includes both planned and responsive support (during working hours) and facilitating sessional support and group activities.
- Liaise, coordinate and collaborate with a wide range of organisations, agencies, internal teams and local community networks to identify to maximise the range of support available to complement the direct support provided.
- Identify support needs and risks for a wide range of vulnerable customers, which may include those displaying challenging behaviours or with complex needs. In conjunction with other services where appropriate, identify within risk assessments and support planning each customer’s personal goals, targets and outcomes to meet needs and address risks.
- Provide support to customers, which may include at times of crisis, responsive support as well as planned support. Where possible, identify strategies to reduce or prevent the likelihood of further needs arising. This may include practical assistance until such time as the customer is again able to cope independently and/or other arrangements are in place.
- Work with others on a multi-agency basis to facilitate and coordinate access to a range of services/networks in a person centred manner, to meet the individual needs of customers and their children. This could include internal teams, adult health and social care, children’s services and other agencies and partners. With a focus on positive outcomes that promote independence, wellbeing, positive interventions and keep people safe.
- To assist in facilitating and coordinating a range of leisure, recreational, educational, health, social and therapeutic activities, both by making connections with the wider community networks promoting the culture of a supportive community support within schemes, including developing schemes as “hubs” for the wider community.

- A full UK driving licence and use of own vehicle.

Experience & Qualifications
Essential:

- Basic understanding of property safety-related contracts and an understanding of KPIs and cost models (e.g., schedule of rates).

- Proficient in Microsoft Office and job management/CRM systems.
