We are seeking a confident and experienced Qualified Social Worker to join Derbyshire's Short-Term Assessment & Reablement Team . This is a fast-paced, front-line position suited to someone who is skilled in handling complex cases and able to work both independently and collaboratively within a supportive team.

Location: Floor County Hall, Derbyshire Dales South P&P Team

Pay Rate: 33-34 per hour (depending on experience, umbrella)



Key Responsibilities:

  • Undertake Care Act assessments and develop person-centred care plans

  • Manage safeguarding enquiries and contribute to MARM processes

  • Complete Mental Capacity Assessments (MCA) and Best Interest (BI) decisions

  • Work directly with service users and carers to promote independence and positive outcomes

  • Participate in Duty cover 1-2 days per week

  • Conduct face-to-face visits as required



Requirements:

  • Qualified Social Worker (Degree/DipSW/CQSW)

  • Current Social Work England registration

  • Previous experience within assessment or reablement teams

  • Ability to work at pace and manage a varied, complex caseload

  • Car driver with access to own vehicle (essential due to community visits)
