Project Worker 1 - Outreach Worker

Would you like to make a real difference to the lives of children and young people with disabilities, and their families?

We are seeking 3 Project Worker 1 - Outreach Workers to join our team! The positions are on a fixed term basis until March 2026, with possible extension.

The service, aligned to Willowgrove - short breaks residential children's home, is commissioned by Southern Health and Social Care Trust, seeks to provide families with a short break from their caring role. The outreach workers will work in families' homes and in the community outside of school hours.

The postholders will provide practical home and community-based supports to young people with learning disabilities . The service will be available to families in the SHSCT locality area.

Duties will include:

  • Working with families on a one-to-one basis to achieve positive outcomes
  • Developing and maintaining a sound knowledge of local provision for disability
  • Working in partnership with parents and carers to achieve better outcomes for their children
  • Carrying out assessments of need and motivate and mentor families to avail of opportunities
  • Applying safeguarding and child protection procedures
  • Working flexibly in line with the service requirements

You will have:

  • Demonstrable experience either in a paid or voluntary capacity of working with children and young people.
  • Understanding of and ability to apply safeguarding and child protection procedures.
  • Knowledge of disability and how it impacts upon children, young people and their families.
  • Experience of timely and accurate recording and report writing
  • Full UK driving licence, business insurance and access to a car for work purposes (this criteria will be waived for candidates with a disability who can demonstrate alternative means of meeting the mobility requirements of the post)

Salary: £25,492.40 - £29,726.27 (full time equivalent).

What will I be doing?

Approved Premises (APs) work with high-risk people released from prison, providing a vital element in public protection arrangements. APs offer a level of contact, support and supervision within the community that is unique within the Criminal Justice System. This is an opportunity to be part of a dedicated, enthusiastic staff team at our male Approved Premises in Battersea, playing a key role in pro-social modelling, rehabilitation and community reintegration for those ready to make positive choices.

Working closely with Probation Officers and other community services such as police, health, housing and substance misuse, we provide a stepping stone from prison into the community.

As a member of the Approved Premises team, you will model pro-social behavior and foster a positive, respectful culture with both clients and colleagues. You will actively engage with residents, supporting communal activities such as meal preparation, and contribute to the delivery of individual sentence plans in collaboration with probation officers, police, and external agencies.

Your role will involve providing administrative and practical support, ensuring daily tasks are completed, and following all operational protocols. You will encourage compliance and enforce rules relating to curfews and drug and alcohol testing as required. Keeping colleagues, multi-agency partners, and management informed is essential.

You will be responsible for monitoring and maintaining the health, safety, security, cleanliness, and well-being of the premises and residents, utilizing tools such as CCTV, observation, and direct personal contact. Accurate, timely record-keeping and appropriate information sharing with respect for confidentiality are critical components of this role.

Using your initiative, you will identify and address additional tasks or emerging issues, working within the service to maintain an enabling environment. The role requires the ability to work both independently and as part of a team, including supporting the induction of new peer staff and volunteers, and collaborating effectively with statutory services. Candidates should have experience planning and delivering added-value initiatives such as events, groups, or volunteer programmes, and be confident in building partnerships across statutory and community sectors. They will bring strong communication, risk assessment, and case management skills, and be capable of representing the organisation at external events, including delivering presentations to stakeholders. The candidate must demonstrate effective time management, the ability to work under pressure, and good IT literacy, particularly in MS Word, Outlook, and internet-based tools. We are seeking a compassionate and proactive individual to provide person-centred support for adults with complex mental health needs. You will engage service users 1:1 to co-produce needs, risk, and recovery plans, supporting them in building life skills, emotional resilience, and community connections. The ideal candidate will assist with resettlement, secure appropriate move-on accommodation, and advocate for fair tenancy agreements. You'll work collaboratively with 
multi-agency partners, families, and carers to ensure holistic, strengths based support. Strong interpersonal, organisational, and crisis management skills are essential.
