ABOUT THE ROLE
As Head of Care you'll use your compassion and experience to make sure our residents get the quality care they deserve. You can expect to work closely with the Unit Manager and Deputy General Manager to identify where we can implement improvements that will make a real difference to our residents' lives. We'll also look to you to ensure that the home is in line with all regulatory and statutory requirements. As well as monitoring the delivery of care, you'll help manage staff in all respects, whether that's by motivating your team, reviewing their performance or planning the rotas. As Head of Care, you'll receive excellent support alongside plenty of opportunities for continual professional development, all within an engaging and fulfilling environment.
ABOUT YOU
You'll need to have an NVQ Level 3 or Advanced Diploma in Health and Social Care to join us as Head of Care. We'll also look for an assessor qualification, or a willingness to undertake one. You'll have supervised others on a senior level and will be able to demonstrate knowledge of clinical care in the older person. As an ambitious individual with excellent communication skills and a can-do attitude, you'll inspire your team to deliver the highest standards. Complex Care Case Manager (Clinical)
£35,000 (negotiable) plus £500 quarterly bonus
Manchester
Full-time, 35 hours per week
We are seeking a motivated Complex Case Manager who is dedicated to positively impacting our clients and promoting a positive atmosphere within their care team. As a leader, you will inspire team members to reach their full potential and provide top-quality, individualized care. Your friendly, supportive, and professional approach will set a high standard for person-centered care. Responsibilities:
- Act as a role model, fostering a supportive environment to empower staff in delivering quality care. li>Uphold internal and external standards, adhering to CQC regulations, contractual requirements, NICE guidelines, company best practices, and Equal Opportunities.
- Take joint ownership of a caseload of complex clients and manage their care package holistically, in collaboration with their appointed Nurse Case Manager.
- Develop, review, and implement client care plans and risk assessments following CQC regulations and best practice guidance.
- Co-ordinate and lead the transition and ongoing continuity of all allocated complex care packages, working with clients, professionals, and Nurse Case Managers.
- Maintain high standards of documentation and record keeping in compliance with CQC, national legislation, and local standards.
- Conduct regular auditing of records and daily evaluation notes, reporting trends and taking appropriate actions.
- Inform the Service Manager of any issues that may impact the quality of care or the health and safety of clients and healthcare workers.
General:
- Comply with the Company's procedures regarding Conduct and Confidentiality.
- Take responsibility for personal health and safety, as well as the health and safety of others.
- Continuously develop professional skills through relevant training and updates.
- Ensure systems and databases are kept up to date.
- Complete any other tasks assigned by the Line Manager within the scope of the position.
