Main Responsibility
  • To implement treatment by following care plan.
  • To keep the patient at the centre of his or her care, ensuring patient and carer participation in decision making.
  • To undertake the daily care and support of delegated patients.
  • Deliver treatment/care within a community setting as specified in a written care plan, under the guidance of a registered practitioner.
  • Wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure that patient receive the appropriate care in the most appropriate setting.
  • Responsibility for the day-to-day care of patients including monitoring.
  • Support the taking of medication under the authorisation of the qualified practitioner.
  • To have a good knowledge of infection control, to reduce risk of health care acquired infections.
  • To enable patients to reach their optimum level of independence.
  • Support terminally ill patients and their carers/relatives.
  • To communicate with individuals about routine and daily activities, this may include overcoming difficulties (e.g. hearing/language).
  • To ensure the needs and wishes of the individual are documented.
  • To work within a multi-professional team
  • To undertake specific tasks as delegated.
  • To assist registered and junior staff, referring to registered staff when necessary.
  • To practice the principles and philosophy of individualised patient care in accordance with policy, procedures and regulatory body’s guidelines.
  • To obtain consent for any treatment undertaken.

The Team  

The Family Safeguarding Adult Team is integrated into Children's Social Care services, providing intervention with parents & carers to actively support change, with the intention to help more children to live safely at home.  

We are looking for a Team Lead to support the Family Safeguarding Adult team and lead the provision of the adult mental health and wellbeing intervention. Responsible to the Service Leader, the post holder will lead the team in all aspects of  practice and development. 

The Role  

You will be responsible for supporting adult specialist workers to assess need and carry out specialist interventions to improve outcomes for families, safeguarding the wellbeing of vulnerable children and their parents/carers. This includes helping to develop skill and confidence in working with adult mental health needs across the  children's social care workforce and to maintain a positive network with existing adult services in Portsmouth.

The ideal candidate  

We are looking for a qualified registered social worker with significant experience and expertise working with parents/carers affected by their mental health and wellbeing. We need a professional who has a comprehensive understanding and appreciation of the value of intervention and support with families, including vulnerable adults who are parents. The Family Safeguarding Adult team will be working predominantly with parents where concerns about their mental health, substance misuse and/or domestic abuse is having a detrimental impact on their parenting ability and capacity to change in order to meet the needs of their children. Some parents may also have learning needs.

You will have/be  

A relevant Social Work qualification and be registered with a relevant professional body.  

Substantial post qualification experience of work with adults who experience mental health and wellbeing needs. Experience of intervention with adults with vulnerabilities associated with substance misuse, domestic abuse, and or learning needs would be advantageous.

Experience of working with adults who are parents / carers and have individual needs that impact adversely on their parenting and capacity to change. 

The skills to be a confident and competent manager and be able to model the values and behaviours that are consistent with a restorative, relational approach. Take an active role in contributing to the development of an integrated model and support the Service Lead to manage any challenges that arise as part of cultural change.  

Confident in applying relevant theory and research to enhance practice. Promote learning and development through providing advice, guidance and support, including facilitating restorative meetings, reflective practice sessions, supervision and training.

 Able to take responsibility for self-directed learning and evidence effective professional growth. Be able to contribute to staff development through coaching and mentoring.  

Able to manage stress and assist others in prioritising work and balancing competing demands/pressures. Committed to staff wellbeing and support staff to develop coping strategies and access appropriate wellbeing services.

Key Responsibilities and Accountabilities:

Following appropriate training, supervised practice and competency achievement assessments, you will be able to undertake the role of Healthcare Assistant (HCA) within the surgery team in ophthalmic (IOL) theatres.


You will also be responsible for the following:


  • Pre-operative testing; conducting tests prior to examination and treatment and assisting during the consent appointment process, including providing information to patients prior to their Cataract procedure.
  • Patient discharging; instructing the patient pre and post procedure on the care of the eyes, updating medical records, making follow up appointments.
  • Co-ordinating: Controlling patient flow on the day of the consent appointment and surgery appointments to ensure minimum waiting times for patients and managing queries.
  • Assisting the surgeon during consent appointments and treatments – acting as scrub assistant, taking notes for the surgeon; instilling eye drops.
  • Undertaking the role of HCA in IOL theatre to assist the surgical team and patients during surgery and associated tasks such as decontamination of instruments.
  • Actively participating in the end of treatment day activities; updating patient files, filing, compiling ‘end of day’ reports for head office, cleaning the department, stock and lens ordering.
  • Reporting untoward incidents – taking an active role in patient safety issues and quality and risk management.

  • Qualified Occupational Health Advisor
  • Full time role
  • Permanent position
  • Salary 49,000 +generous pension and travel to site covered
