Key Responsibilities:
- Provide expert specialist nursing care to patients requiring palliative and end-of-life care, focusing on pain and symptom management, and psychosocial support.
- Conduct comprehensive holistic assessments of patients and their families, identifying needs and developing individualized care plans.
- Act as a primary point of contact and advocate for patients and families, ensuring their wishes and preferences are respected.
- Collaborate effectively with physicians, social workers, therapists, and other healthcare professionals to ensure coordinated and integrated care.
- Offer expert advice, education, and support to healthcare professionals, patients, and families regarding palliative care principles and practices.
- Lead and participate in case conferences and multidisciplinary team meetings to discuss complex patient cases.
- Develop and deliver educational programs and training sessions for nursing staff and other healthcare professionals on palliative care topics.
- Contribute to the development, implementation, and evaluation of palliative care services and policies.
- Engage in research activities and stay current with the latest evidence-based practices in palliative care.
- Maintain accurate and timely patient records in accordance with professional standards and organizational policies.
- Provide emotional and psychological support to patients and their families during challenging times.
- Supervise and mentor junior nursing staff involved in palliative care.
- Registered Nurse (RN) with significant post-registration experience.
- Master's degree in Nursing, Palliative Care, or a related field.
- Advanced specialist qualification or extensive experience in Palliative Care Nursing.
- Proven experience in conducting holistic patient assessments and developing care plans.
- In-depth knowledge of pain and symptom management techniques relevant to palliative care.
- Excellent communication, interpersonal, and counseling skills.
- Demonstrated ability to work effectively within a multidisciplinary team.
- Experience in education, mentorship, and service development.
- Strong understanding of ethical issues and psychosocial aspects of end-of-life care.
- Current NMC registration and relevant professional certifications.
- Willingness to travel between clinical sites as needed within the London area.
