Duties and responsibilities:

• To ensure that standards of patient care are consistently maintained at all times in accordance with agreed operational policies and procedures as a skilled member of the nursing team, ensuring a clean and safe environment.
• To promote and safeguard the well-being and interests of all patients, employees and visitors.
• To assist Senior Staff in their management of clinical departments and to provide effective leadership and management in their absence.
• Assess patients' health and wellbeing across a complex and changing caseload.
• Assist in the delivery of care to meet individuals' health and wellbeing needs
• Provide and receive complex, sensitive or contentious information.
• Acknowledge the risk of Healthcare Associated Infections (HCAI) and understand own responsibility as agreed with line manager in the prevention and control of HCAI
• To maintain standards of infection control within the hospital and own department and to assist patients to maintain their own infection control needs
• Provide and receive patient information, ensuring that all patient information is provided, received and recorded in accordance with the Data Protection Act and Spire Policies.
• Carry out any other duty that reasonably falls within the general nature and level of responsibility of the post

Who we're looking for:

• Registered Nurse with NMC registration
• Previous nursing experience.
• Competence across a range of nursing skills supported by professional and clinical knowledge.
• Possess the mandatory training requirements for role including infection control and health and safety competencies
• Basic workload / shift management
• Able to demonstrate the principles of accountable practice.
• Knowledge of appropriate standards and external bodies, such as the Care Quality Commission

Details of shift pattern:

• Monday to Friday 8-6, Saturdays or weekend work may be required in future

• Current registration as a Registered Nurse (RN) with the relevant nursing body.
• Diploma or Bachelor's degree in Nursing.
• Proven experience working as a Registered Nurse, preferably with experience in community health, district nursing, or home care.
• Excellent clinical assessment, critical thinking, and decision-making skills.
• Strong understanding of patient care principles and best practices.
• Proficiency in electronic health record systems and medical documentation.
• Excellent communication, interpersonal, and empathy skills.

• Up-to-date basic life support (BLS) certification.

• Conduct comprehensive nursing assessments to identify the health and social care needs of individuals.
• Develop, implement, and evaluate individualized care plans in collaboration with patients, families, and other healthcare professionals.
• Administer medications, perform wound care, and carry out other nursing procedures as required.
• Provide health education and support to patients and their families on managing long-term conditions and promoting well-being.
• Liaise with GPs, hospitals, social services, and other agencies to ensure seamless care coordination.
• Maintain accurate and up-to-date patient records in accordance with NMC guidelines and company policies.
• Participate in the on-call rota as required, providing emergency nursing support.
• Adhere to all relevant policies, procedures, and professional standards.
• Continuously monitor patient progress and adapt care plans to meet changing needs.
• Contribute to the safeguarding of vulnerable adults and children.

• Valid NMC registration as a Registered Nurse (RN).
• Proven experience in a community nursing or similar healthcare setting.
• Strong clinical skills and up-to-date knowledge of nursing practices.
• Excellent assessment, planning, implementation, and evaluation skills.
• Proficiency in electronic patient record systems.
• Excellent communication, interpersonal, and teamwork skills.
• Ability to work autonomously and manage a caseload effectively.

• Conduct comprehensive mental health assessments for individuals in acute crisis.
• Develop and implement evidence-based care plans in collaboration with patients and their families.
• Provide direct nursing care, including therapeutic communication and interventions, to manage mental health conditions.
• Effectively de-escalate high-risk situations, ensuring the safety of patients and staff.
• Administer prescribed medications and monitor patient responses.
• Liaise with other healthcare professionals, social services, and community resources to ensure seamless patient care pathways.
