We are seeking an experienced and highly motivated Advanced Practitioner to join our team and provide expert, autonomous practice across our community services. This is a senior clinical role for a registered health or social care professional with a Master's level qualification (or equivalent) in Advanced Clinical Practice and an NHSE 'Advanced' digital badge.
You will deliver high level assessment, complex clinical decision making, evidence based interventions and personalised care for people with diverse and often challenging needs. Working across the four pillars of advanced practice, you will contribute to clinical leadership, education, research, quality improvement and service development.
As a visible clinical leader, you will collaborate with senior managers, APs, Consultant Practitioners and multiprofessional teams to ensure safe, effective, high quality care. You will provide supervision to trainee APs and support staff development within a culture of continuous learning.
You will also play a key role in service innovation, audit, safeguarding, governance, and improvement of patient pathways. A minimum of 20% of your time will be dedicated to nonclinical advanced practice activities.

We are looking to recruit an experienced Advanced Practitioner to support the clinical team in our busy GP Surgery in Redcar.
This is an existing opportunity for a highly skilled clinician to take on a role with autonomy, responsibility, and scope to influence patient care.
Main duties of the jobThis is an autonomous clinical role combining patient assessment, diagnosis, prescribing, and clinical leadership, with strong expectations around service improvement, team support, and safe, evidence based practice.

Key responsibilities:
Clinical assessment of patients with acute, urgent and long term conditions providing subject matter advice.
Independent diagnosis and treatment within scope of professional practice.
Prescribing and management of complex medication regimes.
Risk management and safe clinical decision making.
Clinical leadership, supporting, supervising and mentoring members of the nursing team.
Implement and evaluate individual specialised treatment plans for chronic disease patients.
Domiciliary care - seeing patients in their own homes who are too ill to attend surgery and/or performing a ward round/virtual ward round at our registered Nursing Homes.
Request and collect pathology samples, upon receipt, process and interpret pathology and other tests as requested.
Make referrals to other healthcare professionals or services when clinically indicated.
Recognise, assess and refer patient with mental health needs.
Contribute to Practice targets (QOF, etc), complying with local and regional guidance.
Liase with external services/agencies to ensure the patient is supported.
Deliver opportunistic health promotion where appropriate.
To communicate at all levels within the team ensuring an effective service is delivered.
Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of the individual).
To maintain accurate, up-to-date healthcare records appropriate to the consultation.
Ensure evidence-based care is delivered to the highest standards.
Enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service.
To participate in the audit process, evaluation and implementing plans and Practice change in order to meet patient needs.

Attend and participate in Practice meetings as required.
Ensure clinical Practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
To enable clinicians to safely travel to visit patients in their own home; they are obliged to ensure that your vehicle is in good working order and covered by business class car insurance (any additional premium will be paid by the practice upon receipt of a copy of the vehicle insurance cost).
Contribute to service development, QI projects and practice protocols.
Collaborative MDT working with GPs, Pharmacists, and other healthcare professionals.
Upon request, a percentage of the Advanced Practitioner's time may be spent on personal and practice development activities including audit, research, team development and clinical governance.
Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children.
The may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels.

Provide holistic support in the clinical governance spectrum assuming a senior role in its compliance.
Support the IPC lead for the practice, ensuring compliance.
Participate in local initiatives to enhance service delivery and patient care.

Develop an area of specialist interest, taking the lead within the organisation.
Continually review clinical practices, responding to national policies and initiatives where appropriate.
Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required.
Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation.

- Knowledge and Education:
- Qualified Paramedic or RGN with Independent Prescribing Qualification and clinical skills
- Current NMC registration/HCPC registration
- Evidence of working at an enhanced level
- Skills/Abilities:
- An excellent standard of written and verbal English
- Excellent interpersonal, influencing negotiation and organisation skills
- Ability to make autonomous decisions
- Ability to assess, minimise and manage risks and defuse stressful situations, ability to stay calm
- Understanding the importance of evidence-based practice
- Ability to work within own scope of Practice and understanding when to refer to GPs

- Experience of working in an urgent care, emergency care or primary care setting.

We are looking for a flexible and confident Registered Practitioner to join our fast-paced, forward-thinking service. You will work directly with (often through care co-ordination), and indirectly support, individuals with complex emotional needs. The ideal candidate will be comfortable working within a psychological formulation and utilising the therapeutic relationship to support clients to achieve their goals.
You will be skilled in balancing therapeutic risk taking with robust risk management, incorporating this within your own work as well as supporting others to do the same.
The service often supports individuals who may be diagnosable with a 'personality disorder', as well as those with neurodivergent diagnoses or no formal diagnosis.
You will work within a broader multi-disciplinary team passionate about working with this client group, comprising those with backgrounds in psychology, nursing, social work, and lived experience.
You will have a smaller caseload than is typical in a community mental health setting, allowing you to develop strong therapeutic relationships with those you are supporting and be responsive to both the patient and the wider system.

- Experience of developing others through education, mentorship, coaching, teaching, assessing, presentations, publishing

Advanced Practitioner

An excellent opportunity has become available for a highly motivated, flexible and passionate Advanced Clinical Practitioner to join the Older Adult Home Treatment teams. People currently undergoing ACP training are eligible to apply for band 7 and people who have completed the training are eligible to apply for band 8a.
Currently there are two Home Treatment Teams within the Older Adult Service that the post holder will work across:
- Croydon Home Treatment Team based at the Bethlem Royal Hospital.
- Lambeth, Southwark and Lewisham (LSL) Home Treatment Team based at the Maudsley Hospital.
The post holder will support the clinical team leader in providing clinical leadership and managerial oversight for the allocated caseload, and will support transfers across the Older Adult pathways, taking a lead in clinical decision‑making.
Main duties of the jobThe post holder will deliver, and lead on the delivery of, high quality safe and contemporary care with the whole Older Adult Home Treatment service, ensuring consistently high standards of practice and clinical excellence, including: promoting population health, improving personal outcomes, contributing towards service evaluation, while leading and developing research and quality improvement initiatives.
