We are looking for enthusiastic and capable Band 3 Healthcare Support Workers with relevant experience to join the Adult Complex Care Team where we are committed to delivering evidence-based nursing care to young adults in their own home, following a transition from paediatric services. The young adults may be experiencing life-limiting complex physical conditions and learning disabilities.
The team collaborates closely with parents, families, guardians and other healthcare professionals to ensure that all care needs are comprehensively met, thereby fostering a supportive and inclusive environment.
Candidates are expected to work autonomously and escalate any concerns as necessary. Strong communication skills are essential due to the independent nature of this role. A background in ventilation and tracheostomy management is advantageous.
The successful applicant must possess a valid UK driving licence and be able to drive a variety of patient vehicles.
The post holder will be line managed by Band 5 and 6 Staff Nurses, also based in the young adult's home, and ultimately managed by the band 7 Adult Complex Care Team leader who will be based out-with the client's home.
