Job Purpose:Reporting to the Deputy Home Manager, you will assist in maintaining the highest standard of personalised care and support, supervising the performance of staff in the home and promoting the good reputation of Park House. To professionally lead the care team during any span of duty, ensuring that a safe, effective and efficient care service is maintained which meets the needs of residents. To provide care to residents in accordance with their assessed needs to an industry leading standard. You will enable our Residents to perform activities of everyday living, who, due to a neurological impairment, are unable to perform without assistance. This may include nutrition, mobility, personal hygiene, and support, in line with care plans, whilst maintaining dignity and respect. To ensure that Rehabilitation Assistants receive the training and development opportunities appropriate to their needs and the demands of the service.
What you'll be doing:
Key Responsibilities- In your role as a Senior Rehabilitation Assistant, you will ensure that your team maintains the highest standards of care, support, and rehabilitation by leading the shift effectively and proactively.
- You will maintain the safety of service users, in accordance with national legislation and local policy.
- You will maintain confidentiality in accordance with national legislation and local policy, including GDPR/Data Protection protocols.
- You will supervise team members to ensure continuity of care and adherence to plans and rehabilitation interventions.
- You will take responsibility for staff allocation to Residents.
- You will manage shift rotas to ensure sufficient and safe cover is provided at all times.
- When completing your daily staff allocations, you will enable time for e-learning completion (and any other required training) for all staff.
- You will complete daily administrative tasks, not exclusive to handover notes, resident of the day and Datix.

The Support Worker Role:
We are looking for a Paediatric Rehabilitation Support Worker/Teaching Assistant to support our delightful young 6-year-old female client who has multiple complex needs both at home and within school.
The client requires assistance with all aspects of daily living including; personal care, PEG feeding, administration of medications, applying BAHA hearing device. The client is a wheelchair user although she can walk short distances. She communicates via gestures, signing, some words and has new communication devices which she will need support to use.
Part of the role will be to support our client at school, ensuring she participates in lessons, has assistance in using assistive technology equipment to help demonstrate her learning and understanding, and participate safely in all aspects of school life.
Our client likes relaxing at home with her family, watching Disney movies and her iPad, listening to music and nursery rhymes and loves singing along to them, bouncing on her trampoline, walks in her pushchair, being read to, and her sensory lights.
She loves going to the park on the swing and days out to the farm, sensory rooms/soft play, hydrotherapy, horse riding. Part of your role will be to help her to participate in all these activities and aspects of her life whilst empowering her to be as independent as possible.
We are seeking staff with excellent communication skills and a desire to provide individualized support. Previous care or teaching experience with children is preferred. Staff must be able to participate in a range of activities with the client such as hydrotherapy, and outdoor activities.
Flexible support is required to cover throughout the week in both term time and school holidays, plus overnight shifts.
The successful applicants will receive full training for this role, input from therapists, education and technology specialists and regular supervision by the Case Manager.
Driver with full UK License essential.

About our client: Jack, 52, a long-standing client of Snap Care, along with his case management company, is pleased to be recruiting a new member to join his well-established directly employed team. His nearest and dearest describe him as a devoted husband, a loving father to his teenage daughter, a true gentleman and family man. He has a great sense of humour, enjoys banter and is a big formula one fan. A carpenter by trade, he spends time at his workshop at home working on his craft and enjoys watching movies in his snug room or at the cinema, playing cards, gardening, going to the gym, swimming and painting. Jack has a gym and swimming pool at home. 6 years ago, Jack sustained a Traumatic Brain Injury (TBI), and complex orthopedic injuries; he is a full-time wheelchair user (self-propel and powered wheelchair). As a result of his TBI, he can become fatigued, struggle with short term memory, and be impulsive. He requires support with transfers, either alongside another team member or using a sara-steady transfer aid. Jack communicates verbally, is dedicated to his rehabilitation, and has detailed care plans in place for every aspect of his care, rehabilitation and medical needs; these plans are followed and implemented step by step, by all team members. Jack is a Type 1 Diabetic.
Overview of role: The team provides 2:1 support most days, depending on the activities / plans, and 1:1 care at night. Jack’s wife plans his weekly routine, packed with activities, and this is shared with all his team for the week ahead. As Jack’s Brain Injury Support Worker, you will support him both at home and in the community, facilitate him in his role as husband and father, support his rehabilitation, and enable him to live life as his wishes. We are currently recruiting professionals to join a dedicated 24-hour care team supporting a Jack from Monday to Sunday. The aim is to build a supportive and flexible team to ensure consistent, high-quality care, therefore our client can work around your availability offering set shifts. Who this job would suit : Due to the location, you must be a driver with your own car (limited public transport links available-so this is not an option).
